Procedure To Follow
Please follow the steps below to ensure smooth processing of your LOG.
Step 1
Get your documents in order.
1. Admission form stating medical diagnosis
2. Financial counselling form with
estimated total charges
3. Attending physician statement and procedure to be done (if any)
4. Referral letter from general practitioner or specialist (if any)
Step 2
Submit your LOG request to us via email.
